Medicare Tax Calculator

The uncapped 1.45% line on your stub, worked out per paycheck and per year — plus the extra 0.9% high earners owe and the doubled rate for self-employment.

Your Wages

2026 rates: 1.45% on all wages with no cap, plus 0.9% above $200,000.00.

Why Medicare Never Stops During the Year

Social Security withholding switches off once you reach the annual wage base, which is why some paychecks late in the year feel bigger. Medicare has no such ceiling. Every overtime hour, holiday premium and year-end bonus pays the same 1.45%, and the only movement is upward once a high earner crosses the surtax threshold.

Pair this with the Social Security tax calculator for the capped half, the FICA calculator for both together, and the take home pay calculator for the full paycheck. The Medicare tax guide walks through the surtax rules in detail.
